Offline navigation on mobile Chrome

While dynamic navigation works fine offline on desktop Chrome, it doesn't show up on mobile Chrome.

I'm not sure how to debug on mobile Chrome and figure out what's happening, any help is welcome.

This might be triggered by the following two things:

• Web Publication JS itself is not currently cached and served by the SW
• fetch doesn't work properly in getManifest when offline

Navigation on index.html

In its current version, the JS won't add dynamic navigation if the current location doesn't match something in the spine.

This can be problematic with index.html since / can also see a match for it.
