在线评测测试用例来自于文件不能添加文件

I guess this is not a oj bug. Suggests:

1. Clear your browser's cache and retry.
2. If 1 does not work, perhaps other modules which allow uploading files have the same problem. Right?

不是浏览器的问题，我换了几个浏览器，包括Mac的，都是一样。
其它跟添加文件有关的模块，我都用过了，都没有发现问题。
我印象当中，一开始使用的时候，管理测试用例就是这样的状态，当时没有在意，因为编辑输入没有问题。
孙老师，你有空查查是不是模块本身有什么问题

Can you change the theme to standard and try again? And, if possible, open the javascript debug/console window, there should be some useful information. Thanks.

Sorry, I can not reproduce this bug on my site and have no ideas about it. The last suggest is upgrading both moodle and onlinejudge to the latest version.
