Move middleware into its own repo

Obviously this applies to several projects, so I wasn't sure where to open it. I think it might make slightly more sense to package all of the middleware we use in a repo/gem. I think it would be simpler to simply include it instead of the faraday_middleware gem and mixing our own custom middleware in with the rest of the project files.

I think this would encourage better testing/management of that code, and allow us to create custom middleware (such as error handling) we can share across projects without having to update the same code multiple times.

This recent commit shows how differences in testing middleware compared to API lib code can get slightly messy.

Thoughts?
