Guide instructions

Hi @joelanman

There are a few useful guide instructions that would be good to add.

• VO + B reads from the top of the page to the currently focussed item
• VO + W reads the current item by word, then by letter
• Tab moves to the next interactive element (e.g. input, button or link)

VoiceOver focus boxes are shown

Although the page is hidden, the box to outline the currently selected element is still shown. This makes it harder to read the guide, and adds information a non-sighted user would not have. It would be good if we could hide it.
