[BUG] The bot doesn't ignore quotations

Describe the bug
If someone answers by quoting the original message, the bot will fetch duplicate info of cards

To Reproduce
Steps to reproduce the behavior:

1. Comment an issue by quoting with

[[Island]]

2. The bot triggers even if it fetches again the same cards

Expected behavior
Ignore quoted card references

[BUG] Connections to scryfall need throttling

Describe the bug
If under stress, scryfall will make connections timeout, resulting in (unhandled) error.

To Reproduce
Steps to reproduce the behavior:

1. Comment an issue with 3000 [[Dance of the Dead]] (a wordy card)
2. See error in Actions tab

Expected behavior
Should answer with some sort of warning message or should auto fix the problem.

[FEATURE] Enable fetch syntax in issue titles

Is your feature request related to a problem? Please describe.
If you put [[card name]] inside an issue title the bot can't see it.

It should see also the title text as input
