Remove dependency on Indira

Current code depends on the cwm-indira module, which is deprecated and not part 
of the open-source Cast Wicket Modules.  The code that depends on indira should 
be rewritten and the dependency removed.

Some XML content is not shown anywhere

If the XML file contains content at multiple levels, any text that is a sister 
of a subsection has no place to go in the current scheme.  For instance, with 
<level4> as the page element, a structure like this:

<level3>
  <h3>...</h3>
  <p>Introductory paragraph</p>
  <level4>....subsection...</level4>
  <level4>....subsection...</level4>
</level3>

The introductory paragraph will show up nowhere in the webapp.

In order to get closer to accepting any arbitrary DTBook file, a special page 
should be constructed for this sort of content.

Add support for templates in text responses

Like the template element for drawing responses, text responses should support 
default content specified in the XML file, which would be automatically 
inserted in any new response editors.

page number configuration should be separate for content pages vs. TOC

Right now if isi.pageNumbers.isOn is false, the page numbers don't show up on 
either the TOC or at the top of content pages.  The ones on content pages 
should probably always show up, since they are basic navigation.
