Remove internal singleton pattern

Using an internal singleton pattern to ensure that exists only one instance for each database is error-prone.

Consider the following:

1. Create a LevelDB('dbpath')
2. Remove dbpath from disk
3. Create a LevelDB('dbpath') in write mode

It will not re-create the db, since there is already an instance of LevelDB in the singleton pattern

Improve read only behaviour

If LevelDB is opened in read_only mode, it should raise an exception if the database does not exist.

Currently, it silently creates a database that will be kept empty.

Improve read_only option

If LevelDB is opened in read_only mode, it should not create a new database if it does not already exists.
