I have a similar issue that may be due to the use of the Canibalism knife. I used it at half a heart while standing inside the cube and respawned in a spectator mode-like situation but I couldn't move, change game modes or anything but look around a fixed spot. I quit and re-entered to find I was inside the cube with my grave but all my belongings were still in my inventory and the grave duplicated what I had when I died after I broke it.
