Uncaught ReferenceError: from is not defined:

There was an uncaught error in your index.js JavaScript file line 71 which is causing the form not to work properly. Upon debugging i realized that you had made a typo on the form's id when selecting it on the js file. You can rectify the error by using the correct name you used as the forms id.
