Make GitHub Actions support Azure Credential Separately each App Service

Problem in V1

Each Azure App Service using share Azure Credential, it doesn't work when we want to using Azure Credential separately in each app service.

get-deploy-slot mode will download all Azure App Service app settings (following giving JSON list) and push it into two branches as a plan for preview swap result between slots.

Expected Behavior

Splitting get-deploy-slot mode into 2 modes following below:

• get-deploy-slot mode will download only a particular the Azure App Service app settings. For multiple app services, please use GitHub Actions Matrix to do instead.

• Create new mode, create-swap-plan, it will push the get-deploy-slot results into two branches as a plan for preview swap result between slots.

key `sensitive` in `connectionStrings` section of JSON Settings should be removed

Because connectionStrings is always mask as a secret (sensitive value), the key sensitive will not work if set the value to be false.

After `set-deploy-slot` mode is done, it should update swap plan automatically

If no change in App Settings, skip `set-deploy-slot` in that App Service

Set Slot Setting in only App Service that need to be set, not set all app services

Add more test case

Please add option for custom name in Pull Request

When we using more than 1 environment to swap them, it may make a confuse

The config defaultSensitive is not affect to the Connection String value
