Add links to author guide

Everywhere possible, there should be a link to the relevant spot in the PreTeXt Author's Guide (or Publisher's Guide, if appropriate). If you find something that is not documented in the appropriate guide that you feel should be, then file an issue against the mathbook repository and add a comment here about the topic that could not be linked to the appropriate guide.

If working on this issue during the PTX in PDX workshop, please add a comment here indicating which chapters you've claimed to work on.

Add idx tags

I have thrown in an assortment of idx tags to show the sorts of things that I think should be added consistently and comprehensively. Remove idx tags with great caution, but add them liberally.

index knowls not populating

The links to chapters work, but links to listings or paragraphs don't seem to show. I can see the knowl html files in the appropriate output folder. See below screenshot.

Add axiom-like chapter

And while you are at it, make sure all Entity-Like elements are represented.
