Usage Reports does not properly function w/BR1, BR2, and DB1 COUNTER reports

Originally created at: ndlibersa/usage#14

Back at the end of 2013, a new version of the Usage Statistics module was released that enabled us to import other COUNTER reports besides JR1 and JR1a. These included BR1, BR2, and DB1. In addition, R4 versions of JR1, JR1a, BR1, BR2, and DB1 reports were added. The problem is that none of these reports can effectively be reported on using the Usage Reports sub-module. The Usage Reports sub-module reporting choices are still oriented toward just JR1 and JR1a type reports. With the addition of these new COUNTER reports in 2013, data from them is now intermingled with the "old" JR reports, creating a confusing situation for end users.

Remove Contact/Help link

The contact/help mailto: link does not appear on any other module and does not have any way for user to customized it without directly editing the code.

I think we should remove it.

If we want to add a contact help link to every page of every module in 2.0, then that should be a configuration setting that allows administrators to enter their preferred contact email.

Provide consistency in footer

We need to use consistent footers throughout CORAL modules. Suggest using the same as was recently added to Resources as shown in the attached screenshot. Each module should have the following syntax:

Delete '_DATABASE_NAME_' in the script SQL

Hello,

I think is not necessary to use '_ DATABASE_NAME _' in the script to install this repository because there is a connection established before, if we use this syntax, we create redundancy in the database.
