Thank you for reporting the issue. We have couple of questions:

Are you using "xfs" filesystem for rootfs?
Is this a fresh installation of RHEL7.6 or you have been constantly upgrading since e.g RHEL7.1?

Systems with rootfs formatted as XFS with ftype=0 (default in <= RHEL 7.2) have currently issues with overlayfs2 which we are using in the upgrade process.

Could you please provide output of this command?

Actually, root fs is ext4 formatted.
[root@VM ~]# findmnt /
TARGET SOURCE FSTYPE OPTIONS
/ /dev/mapper/vg0-lvroot ext4 rw,relatime,seclabel,data=ordered

I haven't used the system for days, so I'm not sure the exact os release version the system originally was. It has been surely upgraded, and most probably it was RHEL-7.5. It's server variant with minimal pacakge set, though some development tools was installed later.
