[MW 1.28] Error While Storing

When trying to upload a file, I get the error:
There was an error while storing the file in the stash.

I can't find anything on this error. Is it fair to assume that this extension doesn't take into account bucket location? I'm not on a default server; I'm using US-East-2 (Ohio).

Tested with both public and private bucket directories. The credentials have been verified to work.

Unable to keep record of thumb files

I see that this extension fails to identify in certain circumstances of the existing thumb file on S3 and tries to PUT it again leading to too much load on the server when doing this for big image galleries.

I'm getting the following issue when I try to upload a file from a Heroku instance.

Any ideas what's going on or how to fix it?

Access denied from S3

Not sure what's going on but I just tried setting up this plugin with access credentials that I know have full access to S3 and I'm seeing this when I try to upload:

Any ideas? I'm using MediaWiki version 1.24.2
