Add mnesia tables and setup mix helper

Add tables setup module and its related Mix helper.

must include these tables:

• Person table username, password, profile_info, acl
• Page table title, desc, version, elements, protection
• Element table page, order, type, content
• comment table, page, person, body, date time
• likes table, page, person
• files table, disc_only name, blob, desc

add header and footer to blog UI

header must include login/logout and page title
footer must include links
