Add Admin Metabox for Notes

A metabox that shows the notes (comments) on replies needs to be added.

I haven't thought about it enough to decide if the default comments meta box should be used or if it should be built custom.

Notify all Moderator Participants of New Notes on Replies

Anytime a new note is posted to a reply, all moderators (and above) that are participating in the thread should be notified.

Form doesn't show on lead topics

I was playing with the plugin, and just thought I'd let know that I think I found a quirk with it.

After setting up bbPress to use the lead topic:

add_filter( 'bbp_show_lead_topic', '__return_true' );

The form no longer shows on the lead topic when clicking "Add Note" -- It looks like it's something to do with the ID you're pulling in the add_note_form() method. The form has ID "0" from using bbp_get_reply_id() on what is now the topic. This doesn't match up with the actual topic ID outputted with the "Add Note" link.
