2d-matrix-grid only shows 25 rows

Even though the app settings has an option to specify how many rows to display, the grid only always shows a max of 25 items. In the code the max and page size is set to be 25 but there's no pagination and it's not clear where the app settings are being utilized.

Save button is not always displayed

I reported a similar issue with the Query Counter app. Sometimes when going to Edit App Settings, the Save and Cancel buttons are missing.

Total is not always the last item

Trying to see why/when this happens, it's not 100%, but sometimes when I've got 15+ items in the Y axis, the last Y item is AFTER the total row. This is not hard and fast - I've got one scenario with 21 rows and it renders fine. The column totals are correct and include the values in the last row after the total row.

Items without selected values are not included in the 'No Entry' column

Example: when using Severity as the x axis, there is a column for -- No Entry -- but defects without a Severity selected are not counted.
