Error fixing co-planar degeneracies

Running program on following data gives an error:

1250.000    0.000   25.000        0.00 -100.00  0.00
2250.000   0.000    25.000        0.00 -100.00  0.00
   0.000   2250.000 25.000        0.00    0.00  0.00

When the code tries to build the mesh it sometimes gives the following error:
orientation error: 0x100a04520, 0.000000

(This appears to be a problem when fixing co-planar degeneracies)
