Add support for indexes

I added indexes to some of my entities, like this:

    indexes:
        admin_entity_created_at_index:
            columns: [ createdAt ]
        admin_entity_library_type_index:
            columns: [ libraryType ]

Having them prefixed too would be nice.

Prefix name on not-root inheritance (single_table) is blocked

In PrefixSubscriber, line 45, why your code blocked the application of the prefix table ?
With my schema : Mother Entity, Daugther entity with ManyToMany relation,
Mother has a good prefix
Daugther has a good prefix
RelationTable hasn't a good prefix.
If I delete these 3 lines (45-46-47) every tables are good prefixed.

[Bug] add prefix to specified bundles

Hi I want to add prefix to specified bundles only, but I get the following exception:

Here is my config:

roukmoute_doctrine_prefix:
    prefix:  app
    bundles: ['AppBundle']
    encoding: UTF-8

tag a stable version

I'm having the following warning because I'm using dev-master :

require.roukmoute/doctrine-prefix-bundle : unbound version constraints (dev-master) should be avoided
