After the config is applied, talos gets installed and reboots, it's probably that

[sre42]

For anyone else coming across this issue, I had to include the machine installer configuration to have the guest extensions work after boot.

The image factory provides a boot iso and also an installer configuration image to have guest additions work at boot time and then after talos-install respectively.

For example, I had to add this into my terraform machine config:

  install:
    image: factory.talos.dev/installer/4c4acaf75b4a51d6ec95b38dc8b49fb0af5f699e7fbd12fbf246821c649b5312:v1.6.7

Now the qemu guest additions work correctly and the vm reports the ip.
