Default scope on Device prevents users receiving push notifications

Device currently has a default scope of active: true, which means if a device is made inactive (on logging out of an app, for example), it won't then be found by Device.find_by_token. If the device can't be found, it can't be made active again, and so can't receive push notifications, and a new device won't be created for the user because that would break the uniqueness validation.
