Having the same issue... might be a conflict with optifine of some sort, as I only seem to really get it if I use patcher in conjunction with optifine (not that much tested though)

Tested it a bit further: still happens without Optifine, so it's no conflict.
The log doesn't give much, but when I start the game with Patcher, the log gets spammed with:
The jar file /Users/mrgcgamer/Library/Application Support/minecraft/libraries/org/lwjgl/lwjgl/lwjgl/2.9.2-nightly-20140822/lwjgl-2.9.2-nightly-20140822.jar has a security seal for path org.lwjgl.opengl, but that path is defined and not secure and as it's lwjgl that it is complaining about, that might be the issue? (That error also shows up when the starts successfully with Patcher)

@MrGcGamer I can reproduce this issue without Patcher present, however people seem to say that Patcher makes it more common.

Hm… to me it looks like some kind of race condition, as that happens to me more often, the more mods I have installed and the more RAM of my pc is already used

EDIT: that would apart explain why I thought it is a conflict with optifine… cause it's quite a big mod
