Hosting environment name missing from the logfile

Currently, the hostname isn't populated with any information about which server logged/throw the error.

Include this as an official package

Hi Søren. Thomas, the founder of elmah.io here. Just stumbled upon this package. Can you explain what this packages does? I've been thinking about creating a client for Sitecore for some while. Maybe this repo could mature into an official elmah.io package for Sitecore?

Filter out INFO messages and below

Currently, Elmah.io is being flooded with a lot of INFO log messages. Effectively, this means that even the largest subscription plan available will have a hard time keeping up with the number of logs from Sitecore.

The appender should be restricted by by default to include WARN to FATAL log messages (both included)
