I tried to deploy this tool in a completely internet-disconnected environment in our Organization and it won't work because the images and JavaScript are not available.

Prevent bot submission via captcha

The form is vulnerable to bot submissions. Implement a completely unusable captcha to prevent this.

Maybe solving some type of math problem? Calculus…?

Or questions like “What type of marmot is this?” with a photo of a weasel and a free form text box?

Under no circumstance should the captcha validate.

Suggestion: Download Better Virus'

I'm a little dumb to make a pull request with this feature,
but when downloading a "virus" maybe add a .bat file download that will run shutdown.exe /r /f /t 00 or something similar, because we all know that solves all the known problems in the universe. :)

Edit: did the thing in fork, but does not auto download, auto run, nor download after loading bar is complete.
could probably inspect element on some sketch ass website that has actual virus' to see how it works but i dont care

Seeing that git link is an ancillary item to the help desk itself, I think it shouldn’t commandeer the tab and should instead open a new one.

The greatest accomplishment of human civilization is not featured in this application and this needs to be rectified.

Add help text to describe each form field

Add a question mark next to each field that pops up a helpful description of the field.

Make sure the description is exactly the same as the label.

Form submission progress bar

Upon successfully clicking submit, the form should show a super serious “submitting ticket - DO NOT CLOSE THIS PAGE” page with a very slow moving progress bar.

Any time the tab loses focus, the progress bar should reset. After an hour, redirect to the original form with a validation error and all of the field details jumbled up.

Add dynamic drop downs for business and service areas

Add drop downs so that the ticket can be directed to the correct business and service area. Should be at least three levels deep, and most options should only have one sub-option.
