add interaction terms to bivariate calculations

For functions like plot_bivariate_surface it could be useful to add an interaction effect between the polynomial terms, since currently they're only implemented as fixed effects. This would be especially useful if #2 is implemented.

add `plot_heatmap`

Consider adding a function that plots a heatmap from two functions. This could be extended to cross more than 2 factors. See Schertz et al. (2015) for an example of crossed VOTf0closure duration continua.

We'd be looking to plot Figure 9 at a minimum given two identification polynomial curves (i.e., plot_bivariate_heatmap), and Figure 6 can be an extension (plot_heatmap)

Use `b0` for intercept in `betas` arguments

If betas is able to treat the first argument as the intercept term, then there isn't a need to have separate functions interpolating between curves and between intercepts separately. However this would require changing other functions to follow the same syntax, and changing the helpers to use the intercept a bit differently. Some functions will also need a -1 to index the correct power for the polynomial terms.
