Two completion providers duplicate code actions

When I first tried it, I didn't see duplicate code actions, but that was probably caused by me installing KHA extensions in the same session without restarting vscode. after restarting I would see duplications.
revisiting the issue today I can no longer reproduce it. there was an update of KHA extension where some things were put on a one second delay.
not sure whether the change fully solves the issue or just makes it more unlikely to occur.

Yea, i make workaround in Kode/vscode-kha#43. If you unwrap two setTimeouts and call vsce package to rebuild and install custom kha extension, it still happens. Maybe there is something like clients conflict, and vshaxe picks both haxe clients while waiting for one, idk.

didn't realise it was your patch :)

I'm not really sure what happens. it might be both extensions registering code actions too close to one another, so that they run twice when invoked.
