Protocol Simplification (cleanups)

• Rename reason to message and add code to system.close with the following values:
  • 0x00 unknown
  • 0x01 version not supported
  • 0x02 timeout
  • 0x03 redirect
• Remove system.close-redirect message in favor of system.close with code == 0x03 redirect putting url in message field.
• Remove system.pong and add ack flag to system.ping instead
• Remove kill field from cancel message and leave its meaning (interrupt or rollback behavior) up to implementation or a user.
