Update information about support in Firefox

The Bugzilla bug referenced in the readme has been marked as fixed since November of 2016 and support for the header has been live since Firefox 50.

The readme file should be updated to reflect that change.

Save some memory by defining the middleware function once

This would probably save a bit of memory:

var middleware = function nosniff(req, res, next)  {
  res.setHeader('X-Content-Type-Options', 'nosniff');
  next();
};

module.exports = function nosniff() {
  return middleware;
};

This would only make a difference if you were using the middleware many times...probably not worth it. Anyone have any thoughts?
