Hi Marco,
The minimum required version to run the VSPackage is .NET Framework 4.5.
What OS and SSMS versions are you using?
Can you please share the build numbers?
Can you try clearing the MEF component cache and lets SSMS rebuild it?
That can be done by deleting this folder
%LocalAppData%\Microsoft\SQL Server Management Studio\<SSMS-VERSION>\ComponentModelCache

I have .NET Framework 4.7 installed.
I tried clearing the MEF component cache, no differences.
Please note that I use a non-admin account by default, but even using "Run As Administrator" it doesn't seem to solve the issue, but I'm not logged in as an admin.
I tried several versions of SSMS (2012, 2016, 17.4).
This is the version info from SSMS 17.4

I made several other tests.
The problem seems related to the use of a non-admin user.
If I login with an admin user and then I install the addin from PowerShell as Admin, the addin works.
If I login as a regular user, even if I install the addin from PowerShell as admin, the addin doesn't work (no matter I clean the ComponentModelCache folder).
Now I can run SSMS as admin (even from the non-admin user) and it works. What is strange is that the setup required to login as admin, and runas admin was not enough to correctly setup the addin.
I am not sure about what is the reason for that, I don't have time now to further investigate. However, it should be possible to run the addin from a non-admin user, requiring an admin just for the setup.
